• Scheduling information processed through Calendly, including appointment selections and information you enter in the booking form.

Protected health information

The public website, contact forms, analytics events, and scheduling tools are not intended to receive protected health information (PHI) or identifiable patient information. Where AVM is expressly engaged to handle PHI on behalf of a covered entity or business associate, that work must be governed by an appropriate written agreement, including a business associate agreement where legally required. Describing a workflow as HIPAA-aligned is not a blanket certification of any client’s entire environment.
